Guardian’s Daily Student Health Assessment 

 

School Year 2020-2021 

 

Guardians must do the following health assessment daily before allowing the student to attend school in-person or to board the bus for transportation to school: 


 

• Temperature greater than 100.4 

• New uncontrolled cough that causes difficulty breathing (for students with chronic allergic/asthmatic cough, a change in their cough from baseline) 

• GI symptoms (vomiting/diarrhea) 

• New rash 

• Exposure to a COVID-19 case during the prior 48-hour period 


 

If any of these symptoms are present, the student shall not attend school on the day these symptoms are present. Guardian will need to notify the school of the student’s absence. If the student develops any of these symptoms during the school day, guardian or designee from student’s pick-up list will need to pick the student up as soon as possible.
